Care Coordinator - Remote 🔥



The Wellvana Care Coordinator will support the patient through complex healthcare environments for the optimization of care and outcomes. The Care Coordinator will facilitate patient follow-up through coaching, information transfer, and communication to improve continuity of care and streamline the intervention process. The incumbent guides patients through the health care system by assisting with access barriers, developing relationships with service providers, and tracking interventions and outcomes. The Care Coordinator will serve as a point of contact for referring physicians, patients, and hospitals to provide resources and assistance with accessing clinical and supportive care services.


  • Maintain productivity and quality metrics
  • Provide timely outreach to members newly discharged from Wellvana Preferred Facilities
  • Engage with Wellvana Preferred Facilities for data collection and/or appointment needs
  • Participate in preventive care programs for ACO and Commercial patients to support and care coordinate for high risk and rising risk patients
  • Work with Wellvana clinical leadership to cost-effectively achieve the stated goals and objectives, working towards a mutually greed-upon timeline with the patient’s well-being and outcome as the highest priority
  • Supports closing quality gaps in care
  • Assist with referral coordination
  • Assist office staff with scheduling follow up appointments
  • Assist in routine clerical functions such as filing of medical information in patients’ records using the EMR and obtain medical records from other providers
  • Serve as the liaison to the Care Navigation team, ensuring that patients next steps in the healthcare journey are properly arranged, documented, and followed up on
  • Support Partner practices in Chronic Care Management (CCM)
  • Prepare charts for partner practices to ensure the providers have all the information needed to execute care plans during the office visit
  • Stratify patients based on risk, opportunity, and care gaps
  • Partner with the Wellvana Clinical Team to support programs as appropriate
  • Identifies potential problems involving resources and/or safety and takes appropriate action
  • Understand patient needs and providing a world-class experience
  • Other duties as assigned
. Requirements:

  • Associates degree in Nursing/ LPN or Certified Medical Assistant
  • Certified Medical Assistant or Certified Nursing Assistant/Licensed Practical Nurse preferred
  • At least 3-5 years of clinical experience in a hospital, skilled nursing facility, home health, or physician office setting. preferably in case management is required
  • Knowledge of EMR and healthcare-related patient quality monitoring and reporting technology.
  • 1-2 years of TCM and CCM experience
  • Knowledge of insurance reimbursement and pre-authorization requirements across all payors, to ensure proper placement of patients
  • Proficient in MS Office applications and ability to learn department and job-specific software systems
  • Demonstrate organizational skills
  • Demonstrate effective verbal and written communication skills
  • Demonstrate analytical skills when problem-solving
  • Demonstrate high attention to detail and a high degree of accuracy
