We are currently seeking a Social Service Specialist I to join our Caregiver Support team in Seattle, WA.

What will YOU do?

The social services specialist I (SSSI) will join the Caregiver Support team, and the broader caregiver support network, in supporting unpaid caregivers of adults by administering a comprehensive

evidence-based program (TCARE) to assess caregiver stress levels and care needs and developing a plan to increase support through referrals to relevant programs/services. The position is remote but there is opportunity for in-home visits with caregivers as well as to facilitate caregiver support groups in person or virtually.  The position could also include facilitating Powerful Tools for Caregivers Workshop and representing Sound Generations at in-person community events. Our team recognizes the importance of self-care and provides supportive spaces for group connection and consult. Pay Rate: $25.62 per hour.

Work Schedule: Monday Through Friday, 30 hours per week, opportunity to work remotely.

Responsibilities:

·      Interview individuals, caregivers, and community collateral to assess degree of services

needed, ascertain facts, gather social history, complete assessments to determine need, extent, and direction of intervention.

·      Prepare case file data, documentation and/or reports. Develop and maintain case records which document decisions and provide record of case plans.

·      Provide formal and informal referral service to community resources; case management

services in a multidisciplinary approach to service delivery; counseling to individuals and families that is goal directed, task centered, and time limited.

·      Review and examine cases on a periodic basis to assess client progress and determine continuing eligibility of programs.

·      Develop plan of care with the client.

·      Coordinate services with other agencies. Refer and monitor the progress of referrals to other agencies.

·      Explain program policies to individuals, families, and members of the community.

Requirements:

·      Bachelor’s Degree OR Equivalent.

·      Two years of paid or volunteer experience providing services to aging individuals.

·      TCARE Assessor Certification and experience with Get Care database.

·      Direct experience serving older adults in accessing support services/resources preferred.
